Summary

Cardiovascular diseases (CVD), such as heart disease and stroke, are the leading cause of death in Germany contributing to about 40% of overall deaths and are a major cause of morbidity especially in the elderly. During the first funding period of the DigiHub „CArdiovascular Diseases – Enhancing Healthcare through cross-sectoral Routine data integration“ (CAEHR), we have developed and implemented digital solutions for inter-sectoral data sharing in healthcare and research. The second funding period of "Digital Hubs: Advances in Research and Health Care" will focus on maximizing collective impact of individual projects for cross-sectoral digital networking in healthcare. Utilizing a common "patient journey" approach, the initiative aims to demonstrate comprehensive care benefits while ensuring sustainable and scalable technical solutions. This unified strategy seeks to create lasting innovation in digital healthcare. With the consortium’s strong clinical and methodological expertise in CVD, CAEHR will extend existing solutions and leverage recently emerged legal frameworks and technologies to optimize emergency care in stroke and all-round care of cardiac interventions. Using the gained experience, we will further extend patient data availability and strengthen interoperability in healthcare and research, employing open interface standards and technologies. CAEHR contributes to a dynamic, extensible and widely available digital ecosystem with a new sustainability strategy to collaboratively create non-profit tools where the primary health market will not be profitable in the near future. By further optimizing inter-sectoral communication and data integration, we expect substantial benefits for patients, healthcare providers, researchers and healthcare systems.
